Doctors make bad business people

Posting here a comment I left over at the Entrepreneur School Blog, where Jim Beach talks about the lack of business sense in doctors. If it sounds rant-ish, I apologize.

There are good reasons doctors make for bad business people (and entrepreneurs). The healthcare system is set up where the customer is an insurance company (1) and (2) practicing medicine has turned into a matter of treating symptoms rather than proactive care. In other words, the doctors expect *you* to call them when you have a problem because on the front-end (the preventative end) they largely have nothing to offer.

Case in point: medical students only take maybe a semester’s worth of nutrition. Insane when you consider that diet (and lifestyle) choices are certainly the most likely causes of people’s health maladies — so really, when it comes to having the proper tools to help patients avoid getting ill in the first place, doctor’s aren’t equipped. They are proactively/preventatively neutered.

I won’t get into the insurance company angle, but that has an impact, too. The customer has been utterly divorced from the healthcare system when you’ve got so many layers between you and your care:

you -> employer’s HR department -> [the insurance company bureaucracy] -> [hospital bureaucracy that negotiates reimbursement amounts] -> doctors who code procedures and provide you care

Simply cutting out the employer side would immediately get the consumer more plugged in as you’d have 10s of thousands of more individuals interested in getting better rates/plans from insurers than the lackadaisical corporate HR departments. I digress.

And there’s a (3) here, too. That’s that the doctor’s go to school for longer than any other profession, which makes them prone to dogmatism/procedure instead of being creative or driven to trial/error and problem-solving.

All things considered, it’s a disaster of a system. It’s no surprise it’s only getting worse. The only surprise (to me) is that so few people are talking about these core, simple problems that are making for such a disastrous healthcare system.

Chance Wins

This bit by Seth Roberts reminds me all at once of Nassim Taleb’s work (Status quo, the fed turkey, works until it completely implodes), Seth Godin’s “This is Broken” idea, and the dinosaurs going extinct.

Who is flying this plane? Do they really know what they are doing? Does chance win over purpose?

Seth’s blog post is centered around the broken U.S. healthcare system — a system which suffers both from a burgeoning status quo as well as no means of introducing alternative solutions. In other words, it is entrenched.

And just like the dinos, when that entrenchment ultimately leads to social upheaval, the failure may be catastrophic.

A robust system must be dynamic.

Our system of human development is broken

When I read this latest from David Friedman, I couldn’t help but think of one word, “broken.” There are so many people, myself frequently included, who are wasting their lives doing things they loathe. Meanwhile, they engage in hobbies, “other worlds” where their talents and energies are spent doing things they enjoy.

What we have is a system that tries to make widgets out of human beings. When the human beings inevitably fail to enjoy their particular widget design, they resort to other activities to distract or make their corporate lives bearable. Everyone loses in this system because people are not deployed to their highest and best (and most fulfilling) use.

The system is broken.

I was reminded of this recently when someone I know in WoW as an unusually competent and charismatic leader, organizer, and player, mentioned the problem of “parental agro.” He is apparently a college student, possibly a graduate student, living with his parents. Older examples are friends in the SCA of whose abilities and energy I think highly, who made their living as school teachers or secretaries or the like—respectable jobs, but not particularly high status or high paying ones.

The pattern is not entirely surprising. It makes sense that an energetic individual who doesn’t find much outlet for his energies in his current career will direct them towards his hobbies. Adam Smith long ago observed that, in the British universities of the time, a professor got no benefit by doing a good job of teaching, since the professors were on salary rather than, as in at least some of the Scottish universities, paid by the students. He concluded that if the professor were naturally energetic, he would spend his energies doing something that might be of some benefit to him rather than doing his job, which would not. Nowadays we call it “consulting.”

Acheiving the HGH Flush in a Workout

Rusty over at Fitness Black Book talks about how to know when you’ve reached a point where your workout intensity will have the lingering metabolism-boosting effect. The test is what he calls the “HGH Flush,” which is basically that point where you lose your breath and your skin is warm to the touch.

This is interesting to me for a couple of reasons. One, I think I’ve unwittingly been measuring my personal workout success by whether or not I reached this HGH flush state. I just hadn’t quite articulated it. Two, a part of this that Rusty mentions is not to do too much. It’s here that I think back to my CrossFit experience last summer, in which I put on muscle and lost some fat, as well. Any CrossFitter knows that the workouts typically result in an HGH Flush by design and also are completed in less then 30 minutes. In other words, they are very intense but also brief.

Comparatively, when I was making a concerted effort at additional fat loss earlier this year, I’d occassionally reach the HGH Flush, but overall, I just did more workout volume. Perhaps this is why I didn’t see noticeable results despite an ample amount of effort (in both IF, diet, and exercise).

How to Tell If Your Fat Loss Workout is the Correct Intensity?

I look for a thing called the “HGH Flush". I forget who coined this term, but this is just an indicator of a good fat loss workout. If your skin is slightly red and hot to the touch and you are out of breath after your workout, then you have achieved the HGH flush. Remember your PE teacher in Junior High making you “run lines” or pushing you until you were out of breath and your skin felt like it was on fire? This is the HGH flush which is an indicator that your metabolism will be increased after your workout and that your body will release a bit more HGH than normal (your body’s fat burning hormone).

Nassim Taleb is no friend of academics

Nassim Nicholas Taleb’s latest from Opacity, no 114 titled “Where is the evidence?” launches into Nassim’s current interest in how we know things given the lack of evidence (That is probably summed up poorly, but it is a focus on the absence of evidence/evidence of absence problem and humanity’s gross ignorance. Taleb mentions the concept in the hour long podcast at EconTalk if you’ve got the time to listen to it).

Taleb’s admonition of academia is brutal — he basically says Ben Bernanke and Larry Summers are “arrogant, formal-thinking civil servants, and Ivy-league semi-retards.” Don’t pull any punches there Nassim!

I’m still not convinced the admonition about negative advice is exactly right, but why quibble? (See prior discussion on Nassim Taleb and Expert Advice)

Finally, I’ve ordered two books by John Gray, who Taleb cites as the “greatest living thinker.” And to think I hadn’t even heard of the guy.

I leave aside the confusion absence of evidence/evidence of absence–and the misunderstanding of the very notion of “empiricism". It is a fact that in the real world of our daily decision-making 1) we do not have much evidence of most relevant things, yet we need to take action; 2) in most situations, “true/false” is never symmetric (one side is more harmful than the other), so the burden of evidence is one-sided. Which is why once these fakes “doing science” lose their tenures after the endowments (and charity) run out of funds, they will be barely fit to do anything in the real-life ecology. I wonder what you can do with an unemployed, say, academic orthodox economist. You could do better with non-post-academic cab drivers. Clearly those the most fit at dealing with “just evidence” will be idiot savants outside their evidence domain.

And I can expect that with the SP500 about 20% lower than here, you will see tenures unexpectedly evaporating. The silver-lining of the crisis, perhaps, with the de-academification of society.

So let me take this into more interesting territory, and express my anti-social-planner views. Even more that in Hayek’s days, the ecology of the real world is becoming too complex for Aristotelian logic: very, very little of what we do can be safely formalized, meaning asymmetries matter more than ever. Which puts the Western World today at the most dangerous point in its history: unless we get the Bernanke-Summers crowd out of there, it will eventually be destroyed by the machinery of arrogant, formal-thinking civil servants, and Ivy-league semi-retards.

Finally, beyond the current mess, I see no way out of this ecological problem, except through that tacit, unexplainable, seasoned, thoughtful, and aged thing crystalized by traditions & religions –we can’t live without charts and we need to rely on the ones we’ve used for millennia.

Creating Communities (or Tribes)

Seth Godin points out that it’s the hallway impromptu meetings (and networking) at conferences that result in real value creation. In other words, the enumerated purpose of a bunch of people being in the same place — the conference, the Sunday church service, the political rally, etc. — is little more than a veneered excuse for humans to come together and socialize in real, meaningful ways.

I say “meaningful ways” because our enumerated causes are often incredibly shallow. I still have great friendships from my old days as a Christian (this was back in my teenage years) even though I am more an atheist or agnostic (whatever) today. Most Christians even within the same church body have vastly different beliefs and still manage to enjoy each other’s company. Why? Because the relationships are bigger than the beliefs. The beliefs are (mostly, or most importantly) just an excuse to get together with others.

I see this phenomenon of all the time, and I’ve often pondered the equivalent of Seth Godin’s question in his post. How do we create a central activity, like church, that ostensibly is the reason for people to gather around, but is in actuality just an excuse for us to get together, have fun, talk, share, build, etc. Religion functions fairly well in this regard but has some unfortunate side-effects (elitism, weird beliefs, dogmatism, true believers, etc.).

There are likely other solutions out there (outside of the dying corporate borg) — other ideas to build communities around — they just need to have a common purpose people can get behind that doesn’t result in too many negative consequences or take itself too seriously.

Hmm …

These tribes of people are arguably a more valuable creation than the fish that were caught or the physics that were learned, right?

And yet, most of the time we don’t see the obvious opportunity–if you intentionally create the connections, you’ll get more of them, and better ones too. If the hallway conversations at a convention are worth more than the sessions, why not have more and better hallways?

What would happen if trade shows devoted half a day to ‘projects’? Put multi-disciplinary teams of ten people together and give them three hours to create something of value. The esprit de corps created by a bunch of strangers under time pressure in a public competition would last for decades. The community is worth more than the project.

The challenge is to look at the rituals and events in your organization (freshman orientation or weekly status meetings or online forums) and figure out how amplify the real reason they exist even if it means abandoning some of the time-honored tasks you’ve embraced. Going around in a circle saying everyone’s name doesn’t build a tribe. But neither does sitting through a boring powerpoint. Working side by side doing something that matters under adverse conditions… that’s what we need.

How Humans Play

From Natural Athletics comes a wonderfully descriptive post from Rafe Kelley regarding a recent training day of “parkour” or “natural movement” (a la MovNat or Methode Naturelle), which I think is simply human play. Sit back, read a snippet, and if you want it in full, go to Rafe’s site:

The weather is beautiful, so we decide to make the trip down to Larrabee State Park, an amazing area of beautiful old forests and a rocky beach with formations of Chuckanut sandstone carved into fantastic shapes for climbing by wind and rain. …

[So] we simply go, running along fallen logs, vaulting rails along the trail. Then, deeper into the woods, running downslope, leaping up and over a fallen tree and taking a large gap across a creek–too much fun; had to do it twice, taking the even bigger thirteen foot or so gap on the way back, this one over a waterfall. Downslope again, slipping sliding to the lower creek bed, two step tic tac off a fallen log across the stream and continue on upslope, which ends with a chest high rock–vault on top, keep running. Upslope again, boulders strewn across, pull on them to gain leverage, a vault here, a jump there, finish the slope, wait for Dane to catch up. More slope ahead, dirt trail, fallen logs, boulders; perfect.

Time for a race, a no holds barred race, inspired by Teghead–a traceur from the UK. We’re off, both go for the pull on the arms, hands in the face, spinning–Dane’s getting in front, I dive for his leg, pull him around, clinched up almost falling down slope, I have to avoid the log, break, he’s spinning–shove–I am in the clear, put that rock between him and me, almost there–aha, top of the slope. So fun, but so tiring.

We catch our breath, looking out down the beach. There is a point sticking out 800 yards away as a crow flies, maybe a mile overland, rocky beach the whole way. Is our training only for sprints I ask myself? This what I have needed to do, a real run. “Lets go here to there,” I say, “No stops.” We’re off, moving smooth at first, scrambling over the rocks, hugging faces, ocean inches away. Quick traverses, spinning around obstacles, up and through holes in the rock, vaulting up to boulders, balancing on driftwood, a quarter of the way…Tired. Dane passes me, he isn’t slowing down. Quads burning, no more power moves, I’m struggling just to run. Watching my feet, gotta keep running, but have to stay safe, every foot placement has to be secure, 100% focus. Dane is out of sight. I round the bend; he is climbing the last face, the last cove is between me and him, just small rocks and then the end. I want to slow to a walk so bad–doesn’t matter, have to finish–watching every foot placement, every foot placement, slippery rocks in the stream, can they be trusted, jump, jump, I guess so. We are almost done, roots hanging from the slope, traverse and ascend, I pull myself over the final lip. There’s Dane looking out at the water. I lay down. He wins this round.

We’re moving on, tide going out. Sandy beach now. We find some rocks–big rocks–the biggest one we can move. It looks like a shark head, maybe some 200 pounds. Full squat, bear hug, lift and carry. Dane’s turn; it doesn’t come off the ground–a point for me. We find some rocks, throw them, press them, carry them. Dane finds a big one, clean and press. Damn, that looks heavy. My turn. I press, get stuck in the middle, can’t finish; bring it back down, split jerk, full overhead, unstable, bail out from under it.

Onwards, almost to the end. We come to a big sloped wall. Amazing horizontal wall run; my feet slip when I try to hug it too close. Next time, even though I know this, am telling myself ‘Lean out, lean out.’ It’s damn scary, but it works. Every foot placement is solid and I’m running. High drop to the ledge after the wall run is awesome. Dane is climbing. Climb up 70 feet off the ground. We look out. So beautiful: San Juan island in the west, tip of the snow-capped Cascades in the south. Finally, we make it to Clayton Beach for big kongs over a massive rock into the sand to finish the day, climb up the last rock and watch the sunset.

I’ll be sore for a week.

James Hogan's Four Principles for Effective Dieting

James Hogan recently posted his “four principles for effective dieting,” which were developed from his personal experience with dieting. It’s a great read on what has worked for James (and as he says, this is a nine-year trend in the making). Comparing notes to my own experiences (Better Health via IF and Low-Carb), I see a lot of overlap. In particular, regular measuring, incremental change, and intermittent fasting (which somewhat translates into Johnson’s Up-Day Down Day) all worked well for me. Of all of those things, I think fasting is the best way to improve your habits because it so starkly breaks rank with existing eating habits.

I also really liked how Hogan articulates targeting the “Healthy Stretch.” I liken this to falling into the dieting “zone.”

Anyway, it’s worth the read in full, so I’m just going to excerpt James’ the four top-level principles:

  • Manage your motivation — “So, the most important thing you can do while losing weight is to successfully manage your motivation.”
  • Use data — “Measuring and using data helps us in two ways. It helps us understand the consequences of our actions more accurately than we’d otherwise be able to. It’s also motivating.”
  • Try different approaches — “Try different approaches. Observe which pieces work (and keep them) and which don’t work (and discard them). Don’t become disheartened if an approach fails …”
  • The alternate-day diet — “The structure of the diet is that you eat a very low number of calories some days (20-50% of the amount required to maintain your weight – your “break even” amount), and a higher number of calories on alternate days. If you’re trying to lose weight, you might target closer to 20% on the “down days,” and if you’re trying to maintain your weight, you might target closer to 50%.”

Finally, I like noting that I know James through Patri through the Seasteading Institute. I suppose I know Patri from having read David Friedman’s blog originally and stumbling into Patri’s works on the old Catallarchy site. What’s fun about these somewhat unpredictable connections is that my old childhood (and adult friend - he was a groomsman of mine) friend Andy happened to be the primary creator/writer for Johnson of the UDDD. The internet: making the world a smaller place a day at a time.

The Anti-Authority Authority [A Doctor's Disclaimer]

I’ve begun reading Robert “Bob” Sears’ The Vaccine Book. Despite what you might be thinking, the impetus to begin learning more about baby (and child) vaccine’s actually came from my wife. I’m not always obsessed with needing for info!

Having just started the book (Just finished reading Stumbling on Happiness), I was immediately struck by a paragraph in the Preface which not only plays the skeptic to doctors generally, but also disclaims the authority of the writer. It’s this anti-authority slant, where someone who is perceived as an authority casts doubt on himself and other perceived experts, that I find so important*. In the field of medicine, where the egos of doctors are bigger than the size XL scrubs they so frequently don, this sort of disclaimer strikes me as particularly unusual, but nice to see!

Some people feel that vaccine books aren’t necessary; after all, why not just ask your doctor if vaccines are abolutely necessary and safe and leave it at that? It takes all of one minute, then you’re done. no research or effort on your part is needed. Here’s the problem with that approach. Doctors, myself included, learn a blot about diseases in medical school, but we learn very little about vaccines, other than the fact that hte FDA and pharmaceutical companies do extensive research on vaccines to make sure they are safe and effective. We don’t review the research ourselves. We never learn what goes into making vaccines or how their safety is studied. We trust and take it for granted that the proper researchers are doing their jobs. So, when patients want a little more information about shots, all we can really say as doctors is that the diseases are bad and the shots are good. But we don’t know enough to answer all of your detailed questions about vaccines. …

Even though vaccines are important, you as a parent are still entitled to know what you are giving your child. you have a responsibility (and a desire) to make informed health care decisions for your family.

*Notably, a real slick charlatan knows the importance of this disclaimer, too, so it’s by no means an “all clear” indicator that the disclaiming expert actually knows anything.

Ignoring the Importance of Fermented Foods (Inuit Paradox)

Seth Roberts dug up some 1935 research that discusses the prevalence of fermented fish and oils in the Eskimo/Inuit diet. That research in combination with the observed “Inuit Paradox” (The Inuit diet consisting almost entirely of meat and Omega-3 rich fish fat) incited further research into the cardiovascular benefits of Omega-3 fatty acids. Roberts point, which he’s made before regarding Weston Price’s findings, is that little attention has been paid to the fermented aspect of these ancient human diets.

As someone who regularly reads numerous blogs that discuss evolutionary fitness, diet, paleo-diets, etc., I can attest that Roberts is right: the fermented food angle is overwhelmingly ignored by people who should know better, with the notable exception of Stephen at Whole Health Source (Maybe one or two others have mentioned fermented foods in passing, but it is overwhelmingly given short shrift).

When I say the Paleo/evohealth pundits should know better, I mean that it just makes intuitive sense (whether the back-fitting story is ultimately true or not) that, prior to refrigeration and other modern food preservation technologies, human beings would have been forced to eat fermented foods. This would be for no other reason than the fact that (for example) a band of humans probably couldn’t polish off a wooly mammoth in one sitting. There would be leftovers; and no way were these hunter-gatherers going to let that hard-earned food go to waste!

Apply the same concept to fruits and vegetables ripening at a certain times of the year as well as other food-timing problems and you reach the unavoidable conclusion that human beings must have regularly eaten rotten or semi-rotten foods.

I suspect Seth Roberts is on to something.

[At first, Stefansson didn’t want to eat decayed fish.] While it is good form [in America] to eat decayed milk products and decayed game [well, well], it is very bad form to eat decayed fish. . . . If it is almost a mark of social distinction to be able to eat strong cheeses with a straight face and smelly birds with relish, why is it necessarily a low taste to be fond of decaying fish? On that basis of philosophy, though with several qualms, I tried the rotten fish one day, and if memory serves, liked it better than my first taste of Camembert. During the next weeks I became fond of rotten fish.

So Eskimos ate fermented whale oil and a lot of rotten fish. (”A lot” because if they didn’t eat a lot of it, Steffanson wouldn’t have felt pressure to eat it.) I had no idea that Americans used to eat decayed game.
